Pre-Ballet through 7th graders > $20.00 deposit
Jazz and Tap class requires a total of $40.00 deposit.
Pointe classes require a $20.00 deposit.
Advanced Ballet classes require a $ 20.00 deposit.
Hip Hop classs require a $20.00 deposit.

Please make any corrections to your dancer's name, as you would like it printed
in the program and on the back of the Recital T-shirt.
Please make a Check mark on the okay line if name is okay.
If name is incorrect – please print corrections.
